骨关节炎(OA)的特征是软骨进行性退化、软骨下骨改变以及过度的骨质增生1。目前,骨关节炎已成为全球范围内导致残疾和经济负担的主要原因之一2。现有的治疗方法主要使用甾体或非甾体抗炎药(NSAIDs)来缓解疼痛和炎症。然而,NSAIDs 常常引起胃肠道和心血管系统的不良副作用3。关节置换手术提供了一种替代方案,但约有三分之一的患者并未获得显著的疼痛缓解或功能改善4。随着对骨关节炎分子机制的理解不断深入,已出现多个有前景的治疗靶点,使得治疗更加精准和个体化成为可能5。鉴于人类蛋白质在生物过程中的关键作用,它们成为开发骨关节炎药物的重要候选靶标。Nelson 等人提出,选择具有遗传学支持的靶点可能有助于解决药物在临床研发中高失败率的问题6

孟德尔随机化(Mendelian randomization, MR)是一种利用全基因组关联研究(genome-wide association studies, GWAS)中的单核苷酸多态性(single nucleotide polymorphisms, SNPs)作为工具变量,推断暴露因素与结局之间因果关系的遗传学方法7,8。与观察性研究相比,MR 利用受精时基因型的随机分配,减少混杂偏倚和反向因果关系的影响9。敏感性分析有助于校正多基因多效性,从而得出接近因果关系的推论。由于这些优势,MR 越来越多地被用于识别多发性硬化症和类风湿关节炎等疾病的治疗靶点10。然而,基于 MR 的预测仍需通过实验验证,而转录组学分析可帮助优先筛选关键的疾病相关基因,从而促进这一过程。

我们获得了南昌大学第一附属医院 biomedical research ethics committee 的伦理批准和知情同意。伦理编号:(2025)CDYFYYLK(08-007)。

MR 分析

数据获取
血浆蛋白质数量性状位点(pQTL)数据来自 Zheng 等人的研究14,该研究整合了五项全基因组关联分析(GWAS)数据集15,16,17,18,19,以及 Ferkingstad 等人的研究。数据的纳入标准如下:(i)全基因组显著性关联(p < 5 × 10⁻⁸);(ii)将血浆蛋白质作为骨关节炎(OA)的潜在治疗靶点。研究设计总结于图1。首先,我们利用 IEU OpenGWAS 的 GWAS 数据以及 Zheng14 和 Ferkingstad20 研究中的血浆 pQTL 数据鉴定候选治疗靶点(补充表 S1补充表 S2)。随后进行 Steiger 过滤和表型扫描,以验证结果的稳健性。IEU OpenGWAS(https://gwas.mrcieu.ac.uk/)用于获取髋关节或膝关节 OA(n = 417,596)、膝关节 OA(n = 403,124)和髋关节 OA(n = 393,873)的汇总统计信息21

SNP 筛选命令

在进行孟德尔随机化分析之前,对全基因组显著性SNP(p < 5 × 10⁻⁸)进行了连锁不平衡剔除处理(r² < 0.001,F统计量 > 10,窗口大小 = 10,000 kb)。

MR 分析

为探究潜在的药物靶点,采用血浆蛋白作为暴露因素、骨关节炎(OA)作为结局,通过 R 软件(v4.3.1)中的 "TwoSampleMR" 软件包实施孟德尔随机化(MR)分析。当某个蛋白仅有单个蛋白数量性状位点(pQTL)可用时,采用 Wald 比值法;否则使用逆方差加权 MR 法(MR-IVW),并随后进行异质性和多效性评估。为校正多重检验,采用 Bonferroni 校正法,优先筛选蛋白的显著性阈值为 p < 5.63 × 10⁻⁵。

Steiger 筛选与表型扫描

为评估反向因果关系,我们进行了Steiger过滤分析。结果为“TRUE”且p < 0.05表明不存在反向因果关系。表型扫描使用LDtrait工具(https://ldlink.nih.gov/?tab=ldtrait#home-tab22进行,以检测pQTLs与其他性状的关联。设定的阈值为R² = 0.1,并采用±500,000碱基对的窗口范围。若pQTLs同时满足以下两个条件,则判定其具有多效性效应:(i)全基因组显著性关联(p < 5 × 10⁻⁸);(ii)与已知的骨关节炎(OA)风险因素存在关联。

全表型关联研究

为了考虑基因的多效性及脱靶效应,我们利用阿斯利康 PheWAS 门户网站(https://azphewas.com/)开展了全表型关联研究(PheWAS),该平台包含约 45 万名英国生物样本库参与者的 15,500 个二分类表型和 1,500 个连续性表型23。为尽量减少假阳性结果,阈值设定为默认值。

蛋白质-蛋白质相互作用(PPI)网络

为了可视化MR鉴定出的潜在蛋白质靶标之间的相互作用,我们使用GeneMANIA(https://genemania.org/)进行蛋白质-蛋白质相互作用分析及结果可视化24

富集分析

为了探究生物学意义,我们使用 https://www.bioinformatics.com.cn 的生物信息学工具进行数据分析和可视化,开展了富集分析。

转录组学工作流程

使用RNA提取试剂盒,按照制造商的说明提取总RNA。采用自动RNA质量评估系统检测RNA质量;仅使用RIN ≥7.0的样品。通过无RNase的琼脂糖凝胶电泳(1.5%凝胶)确认RNA质量。使用Oligo(dT)磁珠富集真核生物mRNA;使用RNA去除磁珠试剂盒富集原核生物mRNA。将mRNA片段化(200–700 nt)后,使用RNA文库构建试剂盒将其转化为cDNA。对cDNA文库进行末端修复、加A尾、连接接头,使用DNA纯化磁珠(1.0×)纯化,并进行PCR扩增。测序在高通量下一代测序平台上进行。差异表达基因的定义标准为log₂FC > 1且校正后p值 < 0.05。

网络药理学

为了筛选靶蛋白的潜在药物,我们使用了 BATMAN-TCM(http://bionet.ncpsb.org.cn/batman-tcm/index.php25。采用评分阈值 0.74(LR = 32.5)来选择已知和预测的化合物。中药成分从 TCMSP(https://old.tcmsp-e.com/index.php)中获取,并通过口服生物利用度(OB)> 30% 和药物相似性(DL)> 0.18 进行筛选26

分子对接

采用分子对接方法评估结合相互作用。蛋白质结构从PDB数据库(https://www.rcsb.org/)获取。使用UCSF Chimera通过去除配体和溶剂对结构进行预处理。利用AutoDock Tools计算Gasteiger电荷,并定义对接盒子的中心和尺寸。药物结构从PubChem数据库(https://pubchem.ncbi.nlm.nih.gov/)获取,并进行类似的预处理。对接计算使用AutoDock Vina完成。对接盒子的尺寸根据靶标不同而有所调整。计算结合亲和力,并在UCSF Chimera中对结果进行可视化分析。

筛选骨关节炎致病蛋白的蛋白质组学研究

本研究采用双样本孟德尔随机化(MR)分析方法,用于预测与骨关节炎(OA)相关的潜在蛋白质,并结合OA患者血液样本的测序验证及网络药理学分析,进一步预测OA的药物靶点。我们利用血浆蛋白质组学数据和OA的全基因组关联研究(GWAS)数据进行双样本MR分析,共鉴定出19种与OA相关的蛋白质,包括EFEMP1、PTHLH、PCSK1、ECM1、ISLR2、SEMA3G、SPOCK2、LRIG3、TMEM190、CRYZ、KNG1、OMG、DDX19A、MAPK3、PTPN9、USP8、DDX19B、PROC和ITIH1。为验证MR结果的方向性,进一步实施了Steiger过滤分析。随后进行表型扫描,排除可能通过其他通路影响OA的四种蛋白质(EFEMP1、PTHLH、PCSK1、ECM1)。此外,采用pheWAS分析评估这些治疗靶点蛋白是否对其他性状产生有益或不良影响,并探究未被测量的潜在多效性效应,以弥补MR分析的局限性。同时,对OA患者外周血样本进行了差异表达基因的转录组测序,结果显示31个基因上调,4557个基因下调。将该结果与MR分析结果进行交叉分析,鉴定出五个靶点蛋白(CRYZ、KNG1、USP8、DDX19A、DDX19B)。为深入理解...
